The Additional info form in Teamdash allows you to collect extra details from candidates after they’ve already applied.
When to use the Additional info form
You can send this form to candidates at any stage of the recruitment process. Typical use cases include:
Assigning homework tasks or test assignments.
Asking follow-up screening questions.
Requesting documents or files (e.g., certificates, portfolio samples).
Collecting candidate consents (e.g., background checks, data processing).
Gathering links to personal websites, GitHub, LinkedIn, or portfolios.
💡 Using Teamdash forms, all information is automatically gathered and linked to the candidate profile. Project members can access all needed information from one place.
👉 Forms are sent out using a merge tag called forms_url. We recommend sending out forms through automatic stage actions.
How to create an Additional info form
Go to the Forms section
Choose the form type
Click Create form.
Select the Additional info form from the list.
Give your form a clear name (e.g., “Homework Task – Marketing Role”).
Building up your Form (read more from Introduction on Forms).
Give your form a clear Title (e.g., “Homework Task – Marketing Role mm.yy”).
Choose the fields you need:
Text fields → for open-ended questions.
Multiple choice → for quick screening.
Checkboxes → for agreements or consent.
File upload → for homework or document submissions.
Arrange fields in the order you want candidates to complete them.
Customize settings
Save your form
Once finished, click Save.
Your new form will now appear in the list of forms.
How to send an Additional Info form to candidates
The process is just like sending a message to a candidate.
What is important is to add [form_url] to your message and pick the right form you want to add from the dropdown.
Viewing candidate responses
Once the candidate submits the form, their answers will appear directly in their profile and in the Forms selection, under “View submissions”.
Best practices
Keep forms short — long forms may discourage completion.
Be transparent — explain why you’re asking for additional info.
Use templates — if you often send the same tasks or questions, save the form for reuse.
Track completion — follow up with candidates who haven’t submitted.
