The Forms menu

Teamdash lets you create different types of forms to collect information from candidates and employees.

Types of forms

Candidate Forms

Application form

  • Purpose: Collect initial candidate applications.

  • Use cases: Standard job application form, CV and cover letter upload.

Additional info form

  • Purpose: Request extra information from candidates after they’ve applied.

  • Use cases:

    • Send homework assignments or tests.

    • Collect screening question answers.

    • Request consent for background checks or other processes.

    • Ask for links to portfolios, or other relevant materials.

cNPS survey form

  • Purpose: Measure Candidate Net Promoter Score (cNPS).

  • Use cases:

    • Send surveys to candidates about their recruitment experience.

    • Collect structured feedback on the process.

    • Why it matters
      Candidate experience is crucial to employer branding. With cNPS forms, you can regularly measure how candidates assess the recruitment process and track improvements over time

Employee forms

Internal form

Internal forms are intended to collect feedback from project members on the recruitment process. Read under Project Automations how to automate the internal feedback flow:

Key differences of Form types

  • Application form → First touchpoint, gathers candidate applications.

  • Additional info form → Collects extra details or assignments from candidates post-application.

  • cNPS survey form → Gathers candidate experience feedback.

  • Internal form → Used within the company for structured input from employees.


Creating Forms

Teamdash Form Builder is a powerful tool to streamline your recruitment process. The Form Builder enables:

  • Full customization for creating different Forms.

  • Applying rules on field requirements.

  • Automatic synchronization to the regarding candidate profile

  • Using Standard and Conditional questions types.

Styling

After the field blocks, you can see design options for your application form in the Builder column. You can change the button text, and set colors by adding the color HEX code you wish to use.

When you create a form for your landing pages, we recommend to switch on "Use landing page theme". This guarantees that the form design and colors will adopt to your landing page settings.

After creating a Form to match your organization, you can save it as a template as well.

Forms and GDPR

It's easy to be GDPR compliant while using Teamdash application forms.

  • Static Text field

    • The form must include a reference to your privacy notice for recruitment processes. The privacy notice must be informative. 

    • Add a notice to the label text and add a the public link via the Link icon into your label text.

  • Checkbox Numeric - Months of GDPR consent

    • With Teamdash Forms, you can ask the applicants for a longer data processing consent with the application.

      • by default, the data processing consent is valid until the project end after submission.

      • if a candidate consents to a longer data processing agreement, the profile remains a valid GDPR consent until specific date.

      • the Forms settings must comply with your organizations privacy notice.

The Submissions table

Under the Forms menu, you can see a column Submissions where Teamdash gathers all the submitted data. You can click on the number.

If you have a project target stage set, the profiles will be added to the project too. If you have candidate target fields set for applicant answers, they will become visible on the profile too. But the Submissions table will always be your plan B.

The pink line of code can be used to embed the Form to your website.

Frequently asked questions

I added a new field to my forms. Where will the answers go?

All answers are by default saved in the Submissions table under the Forms menu. In addition, all Form fields have a “Candidate Profile Target Field”. This will determine where on the candidate profile will the sent answers be visible. It may appear as a colourful tag or text in the Comments tab. The possibilities depend on the field Type.

I cloned a form and added it to a new landing page but I can’t see any candidates.

The cloned project doesn't have the Candidate Target Stage set by default. You need to update, where you wish to send new applicants via the cloned form. 

Would you still be able to apply if I mark a project as finished?

Yes, the project status doesn't change the status of your job ads or forms. You would need to make sure the job ads aren't actively up and running. You can quickly navigate to the Job Ads menu and unpublish or archive the job ad.
